- inguinal lig: from ASIS to pubic tubercle
deep ring: at midpoint of ing lig
- inguinal hernia
- indirect: (thru deep ring)
- direct (thru post wall of canal)
- test to determine which:
- after reducing indirect, press over deep ring (mid-point of inguinal lig), ask pt to cough. Should NOT come out if indirect. if pops out = direct
Anmerkungen:
- *in reality, this test meaningless. but need to do for exam
- reduce/originate above pubic tubercle
- other places: epigastric, umbilical
- complications
- incarcerated hernia
- irreducible but blood supp NOT compromised
- strangulated hernia
- irreducible, blood supp compromised
- usuall obstruct as well, so absolute constip
- overly skin color change
- necrotic bowel
- femoral hernia
- thru femoral canal (medial to fem vein, below ing lig)
- reduce/originate below pubic tubercle
- easily strangulates b/c tight ring
- females more likely this kind
- diff diag:
- skin: sebaceous cyst
- subcut- lipoma, fibroma
- arterial- femoral aneurysm/pseudoaneurysm
- venous- saphena varix
- lymphatics- lymphadenopathy
- psoas abscess
- ectopic testis
- hernia- inguinal, femoral
