Fukushima Daiiachi Earthquake and tsunami. Six boiling water reactors(not PWS) of which were operating at the time. All were automatically shut down during this time(takes a long time) emergency generators powering the cooling systems were destroyed during the tsunami. All three reactors experienced a full meltdown. Hydrogen gas explosions blew apart secondary containment structures which stored spent fuel rods. will take years to recover.
