La lidocaína es un anestésico local muy utilizado también como anti arrítmico
Anti arrítmico del Grupo IB,
es un anestésico local tipo amida
con alta afinidad por el canal de Na+ en estado inactivo
MECANISMO DE ACCIÓN
Disminuye la entrada de Na+ durante la fase 2, acorta la DPA ventricular, siendo este efecto más acentuado en las fibras de Purkinje
Normalmente tienen un DPA + prolongado, lo que tiende a homogenizar la DPA e impide la reentrada.
suprime el automatismo y disminuye el periodo refractario efectivo y la duración del potencial de acción en el sistema His-Purkinje
FARMACOCÍNETICA
Se biotransforma en el hígado(metabolismo de 1°paso)
la forma libre como la conjugada de LIDOCAÍNA
son excretadas por la orina

INDICACIONES
Arritmias ventriculares graves que aparecen postinfarto de miocardio, cateterismo, arritmias de la intoxicación digitalica
CONTRAINDICACIONES
-Isquemia regional -Hipersensibilidad a LIDOCAÍNA -Bloqueo cardiáco
ADMINISTRACIÓN
Por infiltración con aguja vía subcutánea, o mediante bloqueos caudales o epidurales.Intramuscular,IV
REACCIONES ADVERSAS
-Colvulsiones,temblor,confusión y alteraciones y alteraciones de la conciencia -Hipotensión, bradicardia y paro cardiaco
INTERACCIONES MEDICAMENTOS
Con anti arrítmicos pueden producirse efectos cardiacos aditivos
