MOA: Indirect Thrombin Inhibitor - - Enhances the effects of Antithrombin III
Indications: VTE, DVT prophylaxis, ACS, line patency
Dose: Patient specific based on indication and aPTT

IV (USED ALL THE TIME)
BOLUS: 80 UNITS/KG (MAX 10,000 UNITS) THEN --> CONTINUOUS INFUSION: 18 UNITS/KG/HOUR (MAX 2,000 UNITS PER HOUR)
SQ: 333 UNITS/KG FOLLOWED BY: 250 UNITS/KG Q12H
ADRs: Bleeding, bruising, decreased platelets
Monitoring: aPTT
Drug interactions: Some - - pt on another antiplatelet/anticoagulant
Food interactions: no
Counseling / Diet: Typically an inpatient medication
