Indicators include: solids pouring over the weirs, excessive floc observed at the surface, and a homogenous (uniform) sludge blanket appearing throughout the clarifier. Sludge in settling test settles slowly leaving a cloudy supernatant with stragglers. What is the MOST probable cause?
