Pre-Employment Screening

[blank_start]No shortness of breath[blank_end], cough, or [blank_start]headache[blank_end]. The patient has a history of [blank_start]diabetes[blank_end], [blank_start]coronary artery disease[blank_end], and hypertension. The [blank_start]physical exam[blank_end] is unremarkable. The patient is resting [blank_start]comfortably.[blank_end]

Patient complains of [blank_start]hernia[blank_end] and a burning [blank_start]feeling[blank_end] down her [blank_start]chest[blank_end]. The [blank_start]patient[blank_end] does not have any history of coronary artery disease.
