Nota:

• Stoffen afkomstig van biologisch materiaal (eiwitten, RNA/DNA).
• Recombinant materiaal is afkomstig van genetisch gemanipuleerde dieren/mensen.

1. Molecular biology
   1. 1. Gen in het genoom traceren
      1. 2. AZ volgorde achterhalen
         1. 3. Polymerase chain reaction: begin en eind stukje van gen door PCR laten lopen en het gen is te pakken
            1. 4. Restrictie enzymen knippen het gen, zodat het past in plasmide
               1. 5. Plasmide in bacterie -> produceert gewenste stofje

      1. cDNA gemaakt uit RNA mbv reverse transcriptase
         1. Middels hybridisatie met een gelabelde probe wordt de gen van interesse eruit gevist

      1. EPO kan niet door bacteriën worden afgebroken, omdat het geglycosyleerd is. Patiënten met te weinig rode bloedcellen of die een operatie ondergaan, hebben hier baat bij.

2. Monoclonal AntiBodies (-mab)
   1. Herkomst
      1. Chimere AB: menselijk AB met Fv van dier (-ximab)

         Nota:

         • AB dier = lichaamsvreemd voor de mens en wekt dus een immuunrespons op
         1. Fv (=uiteinde) dient ter herkenning van antigen/epitoom
         2. Infliximab remt ontsteking (ziekte van Crohn = TNF-a teveel) door TNF-a aan R binding te blokkeren.
      2. Volledig humane AB (-umab)
      3. Gehumaniseerd (-zumab)
         1. Herceptin (transtuzumab) wordt gebruikt bij borstkanker: overproductie HER2-R

            Nota:

            • Bestaat uit 2 delen: herkent kankercel en labelt deze ter herkenning door de immuuncellen.

         Nota:

         • Twee biologisch vergelijkbare gnm, maar niet identiek. Variatie kan ontstaan tijdens het productieproces.
         1. - Biosimilars are not generics, also they are not original biological drugs, but they represent novel third group of advanced biological drugs! - Biosimilars should pass the extremely strong criteria of quality, efficacy and safety by means of CLINICAL TRIALS! (contrary to generics that are NOT applied to clinical testing!)!
