Diltiazem 15-20 mg IV bolus (0.25 mg/kg) over 2 minutes. Additional bolus doses of 20-25 mg (0.35 mg/kg) can be administered 15 minutes as needed. A maintenance infusion of 5-15 mg/hr can be administered for rate control (titrated to the desired rate)
Dopamine 5-20 mcg/kg/min Patient lbs; drop last number and subtract 2 example: 180lbs = 16 gtts/min = 5mcg
Epinephrine 1:10,000 1 mg every 3-5 minutes
Fusosemide (Lasix) 40-80 mg
Lidocaine Bolus: initial bolus of 1.0-1.5 mg/kg; additional boluses of 0.5-0.75 mg/kg can be repeated at 5 to 10 minute intervals until the arrhythmia has been suppressed or until 3.0 mg/kg of the medication has been administered;
Morphine 2-5 mg followed by 2 mg every few minutes until the pain is relieved or until respiratory depression ensues
Nitroglycerin 0.4 mg
Sodium Bicarbonate 1 mEq/kg
Vasopressin 200 mg IM
Lorazepam (Ativan) 0.5-2.0 mg IV; may be increased to 1.0-4.0 mg IV
Diazepam (Valium) 5-10 mg IV for seizures 2-5 mg IM for anxiety
Glucagon 0.25 to 0.5 mg IV or 1 mg IM
Dextrose 50% 25 g IV A second dose of 25 g may be given if needed Pediatric dose mixed 1:1 with saline is 0.5-1.0 g/kg by slow IV bolus
Thiamine 100 mg IV or IM
Benadryl 25-50 mg IV
Magnesium Sulfate 1-2 g over 10 minutes
Oxytocin 3-10 units following delivery of placenta
Ketorolac 30-60mg IM; 30mg IV
Romazicon 0.2 mg IV over 30 seconds; repeat as necessary up to max dose of 1 mg
Narcan 1-2 mg IV
