Inappropriate testing If test plan is inadequate or test data is badly chosen, major errors in the solution may go undetected until system is installed. Also may happen when testing is not allocated sufficient time.
Poorly organised installation and changeover If installation of the new solution does not go smoothly it will affect the attitude of the users and that is not an easy situation to retrieve. If staff training has been inadequate, the system will get off to a bad start and any mistakes made may affect the output.
