Diagnostics History / Examination Vitals, 12 lead ECG, presence of oedema Cardiac enzymes, U&Es, Phosphate, FBC. lipids CXR, CT Coronary Angiogram Stress Test Doppler Ultrasound. Echocardiogram identify risk factors
Left Sided Heart Failure L) ventricle can't pump the blood through to the aorta. L)HF - affects lungs.
Clinical Manifestations of L)HF Dyspnoea/ very SOB Orthopnea Frothy sputum/maybe haemoptysis ?Cerebral Hypoxia Poor Systemic Perfusion Apprehension/anxiety
Right Sided Heart Failure Impaired function of R) ventricle R)HF - affects rest of body.
Clinical Manifestations of R) HF Raised jugular pressure Engorged liver/intestines Ascites Systemic oedema Apprehension/anxiety
Specific Nursing Interventions Weigh daily Strict FBC / Monitor Fluid restrictions Assess oedema/peripheral pulses Monitor vitals Sit upright/Elevate legs Administer O2 as ordered Educate - diet, exercise, medications, disease
Reportable symptoms Persistent cough Increased weakness/fatigue Increased SOB Weight gain > 500 g daily Distended neck veins Fluid input greater than output Changes in trends of vitals
Treatment Low salt / fat / protein diet Fluid restrictions Medications including - ACE inhibitors, Diuretics, Beta blockers Exercise as tolerated
