Why do we administer glyceryl trinitrate tablets (Anginine) sublingually? 1. rapid absorption into systemic circulation for rapid effect 2. avoid first pass metabolism
How should a patient store Anginine (GTN) tablets and why? 1. in original container and tightly sealed as drug is volatile and it adsorbs to plastic 2. away from body- body temperature will lead to loss of drug (volatility) 3. in a cool place (volatility and decomposition) 4. discard 3months after opening (loss of potency)
Why are patients instructed to apply a GTN transdermal patch at 8am and remove at 8pm? this gives patient a nitrate free period and reduces the development of tolerance to this drug
Explain mechanism of action of glyceryl trinitrate nitric oxide released from nitrate activates guanylate cyclase which converts GTP to cGMP and this activates cGMP-dependent protein kinase which acts to reduce intracellular calcium causing smooth muscle relaxation
what is an advantage of GTN as a spray formulation? (nitrolingual spray) if patient has infrequent angina attacks, this preparation has a much longer shelf-life (2 years)
What directions would you give patient on using GTN as a spray use SL prime the first time used or if not used for 7 days
Why do we not give isosorbide mononitrate extended release tablets twice daily (every 12h) to ensure there is a nitrate free time to reduce development of tolerance
