HIV
- Clinical features
- Acute HIV / seroconversion
Anmerkungen:
- Self-limiting, lasts 1-2 weeks and occurs in 30-60% HIV pts
- Sore throat, fever, malaise, arthralgia, mouth ulcer, maculopapular rash
- Symptomatic HIV infection; clues
- weight loss, fatigue, lymphadenopathy, night sweats, diarrhoea
- Recurrent candidiasis
- recurrent / extensive genital warts
- shingles
- oral hairy leucoplakia
- Worsening eczema / psoriasis
- AIDS
Anmerkungen:
- Mean time to progession - 7-10 years, CD4+ less than 200
- Respiratory
- TB
Anmerkungen:
- Early and late HIV present differently - early - normal presentation. advanced - malaise, fever, fatigue weight loss, CXR may be normal or pleural effusion, shadowing. tuberculin may bne negative and smear negative
- Presents atypically; PUO, minimal respiratory features, extra-pulmonary / disseminated disease
- CXR - cavitating lesions but atypical in HIV
- HIV / TB co infection - have a deleterious affect on each other
- pneumocystis pneumonia (PCP)
Anmerkungen:
- Gradual onset breathlessness, fever, dry cough
- exertional desaturation, bilateral crackles
- Cryptococcal
Anmerkungen:
- Lung, meningeal or wound infection. pigeon droppings (neoformans)
- Cryptosporidiosis
Anmerkungen:
- Infects intestine - diarrhoea - potentially fatal in AIDs
- Lymphocytic interstitial pneumonia
Anmerkungen:
- Dry cough, breathlessness
- Malignancy
- Kaposis sarcoma
Anmerkungen:
- HHV 8 virus. commonly presents with lesions on tip of the nose - dermatological condition
- Cervical carcinoma
- Lymphoma
- Systemic NHL
Anmerkungen:
- 30% of malignancy death in HIV - associated with EBV
- Extranodal disease - meninges and bone marrow
- CNS
Anmerkungen:
- usually at CD4 less than 100
- Encephalitis
Anmerkungen:
- CMV
- HSV
- HIV encephalopathy (AIDS-dementia complex)
- Focal / meningeal
- Tuberculous
- Cryptococcal
- CNS lymphoma
Anmerkungen:
- Single enhancing brain lesion - strongly associated with EBV. primary NHL
- Associated with EBV - diagnosis by detection in CSF
- Focal neurological deficits
- Progressive multifocal leucoencephalopathy

- Long term non progressors
Anmerkungen:
- 5-10% HIV patients - little / no detectable virus in blood
- Diagnosis
- investigations once diagnosed
- cervical smears
Anmerkungen:
- Yearly smears as progression to malignancy to rapid
- CD4 count
Anmerkungen:
- Indicates immune status. measure every 3 months
- less than 200 cells /mm - AIDs
- Plasma viral load
Anmerkungen:
- Screen for other STIs
- ELISA - antibody detection
Anmerkungen:
- used in resource poor settings, window period - 3-6 months (need to wait for seroconversion?)
- Combined test - ab and antigen detection
Anmerkungen:
- Window period 6 wks - 3 mnths (can be 15 days)
- Treatment
- HAART
Anmerkungen:
- Generally started at CD4 counts less than 350 or opportunistic infection - balance risks and benefits
- Risks
- drug toxicity
- Immediate - hypersensitivity reaction, acute hepatitis, pancreatitis, lactic acidosis
- Long term; increased MI risk, lipatrophy, lipodystrophy, nausea, diarrhoea
- Drug resistance
Anmerkungen:
- Need compliance of 95% to prevent resistance
- Benefits
- Preservation of immune funtcion
- Decrease risk of transmission
- Prolonged disease free survival
- Reduced mortality from liver disease and malignancy
- Types
- Nucleoside / tide reverse transcriptase inhibitors (NRTIs)
- Non-nucleoside reverse transcriptase inhibitors (NNRTIs)
- Protease inhibitor
- 2 NRTIs and 1 NNRTI / PI
