Pharmacological class anticholinergic = muscarinic antagonist
Indications Asthma COPD Bronchcospasm during or after surgery Use during intubation
Action Promotes bronchodilation by ipratropium binding competitively to cholinergic receptors in the bronchial smooth muscle thereby blocking the bronchoconstrictor actions of the acetylcholine mediated vagal impulses
Metabolism & excretion Partially metabolised in liver, Primarily eliminated renally via active secretion
Half-life 2-4 hours
contraindications/ precautions Contraindicated in individuals hypersensitive to drug or to atropine or any of its derivatives and in those with a history of hypersensitivity to soya lecithin or related food products such as soybeans and peanuts
Possible adverse reactions CNS: dizziness, headache, pain, nervousness CVS: palpitations, chest pain ENT: rhinitis, pharyngitis, sinusitis EYE: blurred vision GI: nausea, GI distress, dry mouth RESP: upper resp tract infection, bronchitis, cough, dyspnoea, bronchospasm SKIN: rash Other: flu-like symptoms
Education - warn patient that med is not for acute relief - Ask patient to show how they use their inhaler
