outflow obstruction in sick infant
coarctation of the aorta
due to arterial tissue encircling aorta @ point of insertion of duct
when duct closes, aorta constricts
-> severe obstruction to left ventricular outflow
commonest cause of collapse due to left outflow obstruction
clinical features
1st day of life
examination: normal
neonates age: 2 days
acute circulatory collapse
duct closes day 2 of life
physical signs
sick baby
w/ severe heart failure
absent femoral pulses
severe metabolic acidosis
Ix
CXR
cardiomegaly
from heart failure & shock
ECG
normal
Mx
surgical repair soon after diag
as for all obstructed left outflow tract
interruption of aortic arch
uncommon
no connection btw prox aorta & distal to arterial duct
so cardiac output dependent on R to L shunt via the duct
VSD usually present
presentation
shock in neonatal period
absent pulses
femoral
left brachial

complete correction within 1st few days of life
closure of VSD
repair of aortic arch
assoc w/ DiGeorge syn
22q11.2 gene microdeletion
absent thymus
palatal defects
immunodef
hypocalcaemia
hypoplastic left heart syn
underdevelopment of entire left side of heart
small/atretic mitral valve
diminutive left ventricle
aortic valve atresia
small asc aorta
nearly always coarctation of aorta
all peripheral pulses absent
circulatory collapse
clinical features
detected @ antenatal US screening
allows for effective counselling
prevents child getting sick after birth
if presenting @ birth
are the sickest neonates w/ duct-dependent sys circulation
no flow through left side of heart so ductal constriction->
rapid cardiovascular collapse
cos no flow through left side of heart
profound acidosis

resus 1st (ABC)
