| HEPARIN - prophylaxis treatment for thromboembolic disorders | Action: anticoagulant - prevents blood clots (thrombins) because heparin binds to and activates antithrombin in the blood which interacts with Factor Xa and therefore inhibits conversion of prothrombin to thrombin (clotting) Side effects: Hypotension, Tachycardia, dizziness, jaundice, nausea, indigestion, headaches and excessive bleeding Route: Subcut Considerations: Monitor HR and BP, Monitor for bleeding, make sure patient is sitting up. |
| WARFARIN - for treatment of blood clots, DVT, pulmonary embolism | Action: Warfarin interferes with the production of vitamin K in the liver which stops blood cells sticking together too quickly, suppresses/slows clotting factors. Side effects: bruising, heavy menstruation, nose bleeds/bleeding gums, pink or brown urine and dark stools Route: Subcut Considerations: Doctor to monitor INR (2.0-3.0) nurse to ensure documentation, monitor for bleeding, monitor BP and RR + HR, do not interchange medication brand name - only use what is prescribed.
