Circulatory changes at birth
  1. fetal circulation
    1. low left atrial Pa
      1. cos relatively little blood returns from lungs
      2. right atrial Pa> left atrial Pa
        1. right atrium receives all sys venous return incl blood from placenta
        2. foramen ovale held open
          1. blood flows across atrial septum from right to left
        3. w/ 1st few breaths
          1. resistance to pulmonary blood flow falls
            1. & blood vol flowing through lungs rises 6x
              1. so left atrial Pa increases
            2. placenta excluded from circulation
              1. so blood vol returning to right atrium falls
              2. change in Pa diff btw RA & LA
                1. -> foramen ovale closing
              3. ductus arteriosus
                1. connects pulmonary artery to aorta in fetal life
                  1. normally closes in 1st few hours or days (day1-2)
                    1. can remain open in some babies w/ congenital heart lesions
                      1. called duct-dependent circulation
                        1. clinical condition will worsen quickly when duct closes
